Special Needs Plans (SNPs) are a type of Medicare Advantage plan designed for people with certain health circumstances. They often provide more coordinated, condition-specific care than standard plans.

C-SNP

Chronic Special Needs Plans

C-SNPs are designed for people with specific severe or disabling chronic conditions. They tailor their benefits, provider networks, and drug formularies to the needs of those conditions.

Common qualifying conditions:
Diabetes
Chronic Heart Failure
Cardiovascular disease
Chronic Lung Disorders (COPD, etc.)
End-stage renal disease (ESRD)
And other qualifying chronic conditions

D-SNP

Dual Eligible Special Needs Plans

D-SNPs are designed for people who qualify for both Medicare and Medicaid (known as "dual eligibles"). These plans coordinate benefits from both programs to reduce or eliminate most out-of-pocket costs.

What D-SNP often provides:
$0 or very low monthly premiums
Coordinated Medicare + Medicaid benefits
Dental, vision, and hearing benefits
Transportation benefits
Grocery and utility assistance (some plans)

SNP enrollment — what you need to know

Who qualifies?

To enroll in a C-SNP, you must have the chronic condition the plan is designed for. To enroll in a D-SNP, you must be eligible for both Medicare and Medicaid. We'll verify your eligibility before you enroll.

When can you enroll?

SNP enrollees have a Special Enrollment Period that allows enrollment at various times throughout the year — you don't have to wait for the Annual Enrollment Period. This is a significant advantage for those who qualify.

We do not offer every plan available in your area. Currently we represent 13 organizations which offer 92 plans in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Program (SHIP) to get information on all of your options.
